Report of Training
It is the program participant's responsibility to ensure this training is documented in the appropriate location according to his/her locally prescribed process. Continuing education credits awarded for participation in the CE activity may not apply toward renewal in all states. It is the responsibility of each participant to verify the requirement of his/her state licensing board(s).
Certificate of Attendance
All participants seeking continuing education credits must have their training summit badge scanned by a room administrator outside each classroom. Participants who do not have their badge scanned will not receive credits.
Note: Credits will be awarded on a per course basis. Please check the Training Summit manual to verify accreditation for each course. It is the responsibility of the participant to ensure a course is accredited for their professional discipline.
Evaluation
Participants obtaining CEs for their attendance must complete all online session evaluations (either onsite or upon return home) and an overall evaluation of the training in order to obtain a CE certificate. Certificates can be obtained at the end of the evaluation process after completing all required evaluations. It is the participant's responsibility to ensure CHEP has your correct email address for sending the evaluation/ certificate link.
