Evaluations
The 2010 Integrated Training Summit evaluation process is now available through this website. In order to access this evaluation system, you will need your confirmation number (name badge or receipt) and please follow the directions below:

Select your training/conference (Integrated Medical, Public Health, Preparedness and Response Training Summit (Jun 02, 2010 - Jun 06, 2010)) from the dropdown menu.

Enter your confirmation number (from your name badge or receipt in your onsite packet).

Select one of the options from the Accrediting body box This is the continuing education choice that you selected when you registered: If you have not requested accreditation please select General Contact Hours from the Accrediting Body Box.
If you require continuing education credits for more than one discipline, hold down the <Ctrl> button on your keyboard and this will allow you to select more than one discipline at one time.

Press Submit.

Take all evaluations.

Click the Print Certificate link.

You may have to wait a moment for your certificate to display. It will display in PDF format so you must have a PDF viewer[1] on your computer.
If you cannot complete your evaluations or cannot obtain your certificate after completing steps above, please contact the CHEP office at (410) 642-1857 or email us at webmaster@chepinc.org.
